EDIT: are you using MySQL for the database? That's the only supported DBMS for this plugin. Postgres and sqlite won't work.
I'm using MySQL. Everything else seems fine. I'm going to try to make a custom field.
Update: I feel enlightened, I was able to make a field without any problems. Now if I could only figure out the board message and award problem, but that is irrelevant here.
Since it worked after deactivating, uninstalling, and re-installing the plugin, I figure something along the lines of cache resulted in a blank page and doing a clean reinstallation fixed it. I had the same problem in an old phpBB forum where I had to delete a cache file for the header just to have it update. I don't understand it myself.
