Subscriptions on index

Hi all,

I've been looking to display thread subscriptions on the index of my website. I was given this code by Yaldaram...but unfortunately it does not work. I made a dummy of what I would like and that is available here. I have spent hours trying to fix/figure this out! But sadly no solutions work . I figured this was the best place to come!
