Conditional custom profile fields

I'm trying to have a second question appear only if the prior question is answered "Yes". This is on the Registration page, under Additional Information. I installed the Template Conditionals plug-in, but I can't figure out where to plug in an IF-THEN that wraps around the conditional question. Thanks!

Thanks! But I still can't figure out where to insert the conditional code, i.e., which template or file does it go in, to control the Registration page questions?

That would be member_register template under member template..
